Procedure

RCP referrals – Paper referrals

• Present an identification document;
• Copies, double-sided referrals, poorly printed referrals and illegible referrals are not accepted;
• The referral must be in good overall condition;
• Doctor’s and health centre’s identification stickers, either digitally printed or affixed;
• Patient’s and doctor’s signatures (digital signatures are not accepted);
• Referral in A4 format;

Valid for 12 months.

RSP format – Paperless referrals

• Present an identification document;
• These referrals may be presented as a printout, by SMS or email, or through the SNS 24 app.
• To accept these referrals, we need the Access Code (Código de Acesso), Service Code (Código de Prestação), SNS user number and/or referral number.

Valid for 12 months.
