Procedure

• Present a valid beneficiary card and an identification document;
• Prescriptions on the clinic’s letterhead or bearing its stamp;
• Doctor’s identification sticker;
• Doctor’s signature;
• Patient’s signature on the front of the referral;
• Digital referrals are accepted;
• Treatment authorisation duly signed by the patient;
• “Especial – portaria 1034/09” – copy of the card;
• Only referrals from doctors registered with the Portuguese Medical Association and from dentists are accepted. Referrals from nutritionists, psychologists or podiatrists are not accepted.

Valid for 12 months.
