Procedure

• Present a valid ADSE beneficiary card and an identification document;
• Prescriptions on the clinic’s letterhead or bearing its stamp;
• Doctor’s identification sticker (affixed to handwritten or mixed-format referrals, or digitally printed on computer-generated referrals);
• Doctor’s signature;
• Patient’s signature on the front of the referral;
• Digital referrals are accepted;
• Only referrals from doctors registered with the Portuguese Medical Association and from dentists are accepted. Referrals from nutritionists, psychologists or podiatrists are not accepted.

Valid for 12 months.
